Laboratory Client Services

The Client Services department is available to provide information and answer questions about tests performed in our laboratories, including test ordering, specimen shipping and handling requirements and test results. Should you have more specific technical questions, you will be referred to an appropriate laboratory staff member or director.

Mycobacteriology Testing

Study Inquiries

For all study inquiries, contact the director of the specific laboratory in which the study will be conducted.

Requisition Forms

Requisition forms are available from National Jewish Health by contacting Client Services or may be downloaded from this website. Please click here to download a Requisition Form.

To assure that your request is processed accurately and in a timely manner, please print or type all information legibly. Incomplete requisitions will delay the processing of your request and subsequent reporting of your results.

Complete a separate Requisition Form for each patient and include it in the specimen package. Before shipping, verify that patient information and patient ID number on the form matches that on the specimen.

Test Results

Test Results can be provided electronically, by fax or via USPS. Generally, sensitive test results such as those associated with HIV are not provided by telephone or fax. Every effort is made to provide accurate test results with rapid turn-around time.
