Online teaching and testing modules about the new medication reconciliation process were created for medical assistants at National Jewish Health. Modules were made available on the employee online learning system and are required for all medical assistants.

Selected modules included short videos showing how to use the electronic health record (EHR) and effective interviewing skills for medication reconciliation:

A video roundtable discussion has also been developed as a resource for other health care providers and institutions who are seeking tools to improve their own medication reconciliation process. It is publicly available on YouTube.


Joint Commission National Patient Safety Goals

Maintaining and communicating accurate patient medication information is one of the Joint Commission's safety goals. Pages 3-4 focus on medication reconciliation and elements of performance in the ambulatory setting.


MATCH Toolkit

The Medications at Transitions and Clinical Handoffs (MATCH) toolkit published by the Agency for Healthcare Research and Quality (AHRQ) is a step-by-step guide to improving medication reconciliation processes in the outpatient and inpatient settings.
