If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.
Download, complete, and print the Request for Amendment of Health Information Form, making sure to include your handwritten signature.

Medical Record Destruction Notice
On May 8, 2024, National Jewish Medical and Research Center, 1400 Jackson St, Denver, Colorado, 80206, will destroy all medical records of persons, Adults exceeding 10 years and Pediatrics exceeding 28 years, who received treatment at National Jewish Medical and Research Center from 02/01/2005 to 02/28/2005. Inquiries concerning records should be directed to the Health Information Management Department 303.398.1580 between 8:00am and 4:30pm, Monday-Friday. Inquiries for the entire record will not be accepted after 05/6/2024.
