Obtaining a Copy of Your Medical Records

Access to medical records is available to patients over the age of 18 or by a legal guardian.

   

Records Are Available in Two Ways:

Request for Amendment of Medical Records

If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.

Download, complete, and print the Request for Amendment of Health Information Form, making sure to include your handwritten signature.

You can send your request to us through fax, mail, or in person.
