Submitting a Sample

To submit a sample for testing:

  1. Choose the appropriate test from the list of available tests below.

  2. Download and fill out a Mycobacteriology requisition.

  3. Package the sample for shipping following the collection/transport instructions for the test on our website.

  4. Send the requisition and sample to the address in the upper right corner of the requisition.
